YATS Applications
Persons wanting to use YATS are required to pre-register by submitting an application form signed by a health care professional. The form includes identification of the user, type of mobility aid used, whether an attendant is required and whether the disability is temporary or permanent.

It is important that YATS staff have up to date information for each registered user. Users are asked to inform YATS of any changes to their application (mailing address, pick-up address, ability information) as soon as possible.
